Job Description

Position Summary: The Client Coordinator plays a key role in managing the client intake process, coordinating services, and ensuring high-quality support throughout the client relationship. This position is responsible for assessing client needs, developing service plans, and acting as a central point of communication among clients, service providers, and internal teams. The ideal candidate is detail-oriented, empathetic, and highly organized, with strong problem-solving skills and a commitment to client satisfaction. Key Responsibilities: Client Assessment & Intake Conduct initial consultations to identify client needs, goals, and service eligibility. Collect and review required documentation from clients. Develop individualized service or care plans tailored to client objectives. Service Coordination Schedule, organize, and monitor the timely delivery of services. Ensure services are aligned with the client’s needs and update plans as required. Collaborate with internal teams and external providers to ensure seamless service delivery. Communication Act as the main point of contact for clients, service providers, and key stakeholders. Provide regular updates and maintain proactive communication to ensure expectations are met. Address questions, concerns, and feedback from clients in a timely and professional manner. Problem-Solving Identify and resolve conflicts or service delivery challenges. Adjust service strategies based on feedback, changes in client circumstances, or service disruptions. Documentation & Reporting Maintain detailed, up-to-date records of all client interactions, service plans, and progress reports. Generate and submit regular reports for internal management and external stakeholders as needed. Compliance Ensure that all services and processes adhere to organizational standards, policies, and legal/regulatory requirements. Stay informed of relevant compliance guidelines and update documentation accordingly. Qualifications: Bachelor’s degree in Social Work, Psychology, Business Administration, or a related field (preferred). Fresher or experience in client coordination, case management, or service delivery roles. Strong organizational and multitasking skills. Excellent written and verbal communication. Proficiency with case management systems, CRM software, and Microsoft Office. Ability to handle confidential information with discretion. Knowledge of compliance standards relevant to the industry (e.g., HIPAA, local regulations).
