Care Coordinator

Responsibilities

1. Provide Doctor/ Health Coach support with chart prep, scheduling etc.
2. Assist clinical teams with administrative member-requests and overflow tasks as needed, including medical record requests and coordination
3. Manage communication between the team, liaising with internal and external executives on various projects and tasks
4. Provide exceptional member service through prompt, accurate, and knowledgeable responses to member inquiries and complaints
5. Maintain ownership of member issues from receipt of the initial request to resolution
6. Follow up with members to ensure their issues have been resolved to their satisfaction
7. Utilize member feedback to identify opportunities for improvement and report trends
8. Manage inbound and outbound inquiries via phone, SMS, email.
9. Ability to think on your feet and de-escalate member situations
10. Listen attentively to customer concerns, empathize with their situation, and provide appropriate solutions and resolutions.
11. Assist new members with clinical scheduling questionnaires, portal questions, and any other coordination.
12. Organize day-to-day coordination, including waitlist management, scheduling.
13. Track and manage clinic-related inquiries, ensuring swift communication between clinical and administrative teams.
14. Proactively reach out to members for feedback post-services to maintain high levels of satisfaction and engagement.
15. Maintain internal processes and coordinate internal and external resources
16. Maintain accurate documentation and records of all interactions and transactions to support efficient workflow management.
