_Key Responsibilities_
1. Assess patients' health and wellness needs
2. Develop and implement individualized care plans
3. Administer medications and treatments as prescribed
4. Conduct health assessments, screenings, and vital sign checks
5. Educate patients and families on health management and disease prevention
6. Collaborate with healthcare providers to coordinate care
7. Maintain accurate patient records and documentation
_Skills and Qualifications_
1. Registered Nurse (RN) licensure
2. Clinic or outpatient experience preferred
3. Strong communication and interpersonal skills
4. Ability to work independently and as part of a team
5. Electronic medical record (EMR) system experience
_Work Environment_
1. Outpatient clinics
2. Primary care or specialty clinics
3. Community health centers
