Should be flexible to do both Transition care and Home care services. Monitoring Pulse, Measuring Blood Pressure, Assessing Respiration Rate, Checking Temperature. Assist patients and caregivers with educational needs, problem resolution. Administering sponge baths to patients, Accompanying the patients for walk, Repositioning, attending diaper changes and the patient’s toileting needs. Ensuring timely administration of prescribed medication. Develops patient care plans. Assists patients and caregivers with educational needs , problem resolution. Providing ongoing evaluation of the quality and effectiveness of the educational activities. Complete other tasks as and when assigned. Follow company protocols. Observe patients and report any changes in patient conditions to the nursing staff or doctor on call. Traveling to patients’ homes and managing their care plans according to physicians’ instructions. Inspecting wounds, changing dressings, and handling personal grooming and hygiene.
