We're hiring for HCC medical coders !!

Role & responsibilities

Medical Record Review

• Review physician documentation, medical records, and charts to identify diagnoses that impact risk adjustment.
  
• Ensure that all conditions are accurately captured and supported by the documentation.
  
• Abstract codes for chronic conditions, comorbidities, and other reportable diagnoses.
  

ICD-10-CM Coding

• Assign accurate ICD-10-CM codes for all relevant conditions according to official coding guidelines and risk adjustment models (e.g., CMS-HCC, HHS-HCC).
  
• Ensure all documented conditions meet MEAT criteria (Monitoring, Evaluation, Assessment, Treatment) for HCC coding validity.
  

Risk Adjustment Compliance

• Apply knowledge of CMS and HHS risk adjustment guidelines to ensure compliant coding.
  
• Validate that codes are appropriately mapped to HCC categories for accurate risk score calculation.
  
• Identify potential overcoding or undercoding issues and report discrepancies.
  

Query Providers

• Prepare and send clinical documentation improvement (CDI) queries to providers when documentation is unclear or incomplete.
  
• Collaborate with healthcare providers to improve documentation quality and coding accuracy.
  

Audit and Quality Review

• Participate in internal or external coding audits.
  
• Review coding output for accuracy, completeness, and adherence to regulatory guidelines.
  
• Implement audit findings and suggestions for process improvement.
  

Data Management & Reporting

• Document coding results in appropriate software systems (e.g., EHRs, coding platforms).
  
• Maintain detailed records of coding decisions and rationales.
  

Education and Training

• Stay current on coding updates, payer guidelines, and regulatory changes.
  
• Attend ongoing training on HCC coding and compliance.
  
• Educate physicians and staff on best practices for documentation supporting HCC coding

Preferred candidate profile

Candidate with min 6 months of experience to 3 years of experience

Non certified is not preferrable

Immediate joiners

Willing to do Work from office
